Application Process for Boards, Commissions or Committees

Thank you for expressing an interest in serving on an Advisory Board in your community.

If you would like to be considered for appointment to any of the Boards, Commissions or Committees, please follow these instructions:

  1. Print the Aplication Form for Boards, Commissions or Committees (PDF: 40kB)
  2. Complete the application by typing or legibly printing the requested information
  3. Return the completed application, with professional resume (if available), to the following address:
